ContiCare is a new free service available to Ventura and Los Angeles County senior citizens who can no longer live alone. ContiCare helps guide seniors and their families through the difficult process of selecting a nursing facility or another care setting such as an assisted living or a residential care facility.
ContiCare also follows clients to make sure that the admission process to the facility goes smoothly and that the senior feels at home in his or her new setting.
ContiCare services start with a personal consultation to determine the client's needs - financial, spiritual and social as well as medical - and to discuss different options for care.
ContiCare helps clients and families contact and work with the facility that they select to provide care. If the client is not familiar with local facilities, ContiCare also makes recommendations tailored to the individual's unique needs and circumstances. Clients may rest assured knowing that ContiCare only makes referrals that meet the company's high standards. ContiCare staff visits each facility personally before making a referral.
The backbone of ContiCare, according to founder Mariana Conti, is the personalized service it offers. Whether a client is suffering from a chronic condition like Alzheimer's dementia or an acute condition like a fractured hip, ContiCare offers appropriate referrals and follow-up to ensure that the person is cared for in the most suitable environment.
Conti, a licensed Brazilian psychologist with more than a decade of experience working with seniors, says that ContiCare is a dream come true for her. "Since I was a kid, I have always had the heart to help seniors," she says. "I feel that I am a catalyst for change, trying to help people at no cost to them. This is my mission."
